The clinical study on 24 cases with duodenal obstruction

Abstract
The present report is a study of 24 cases of-the duodenal obstruction at the Dept. of-General surgery of N.M.C. during 14 years period, from 1959 to 1973.
The cases due to peptic ulcer were not included.
The results obtained are as follows;
1. Among 24 cases of this series, 10 cases (41.7%) were due to congenital anomalies and, 14 cases (58. 3%) due to acquired lesions.
2. Of 24 cases with duodenal obstruction, 12 cases (50%) were due to intestinal, and mesenteric Tbc, 4 cases (16.6%) due to annular pancreas, and 3 cases (12. 5%), were.: arteriomesenteric obstruction.
3. The majority of the cases, `congenital group were found within 1-yea old, and acquired group were 4th decades.
4. The ratio of male and female was 1. 4 : 1.
5. Although the duration of symptoms aiid`signs depend to leading causes of-duodenal, obstruction. The average duration were One and 5 month in intrinsic groups and 10 months in extrinsic groups, was much shorter in duration of symptoms than acquired groups.
6. The most frequent symptoms was nausea and vomiting, occuring in all cases, and the majority of these cases was associated with upper abdominal pain.
7. Upper G-I series were taken in all cases which showed a markedly dilated stomach and proximal duodenum with narrowing or stenosis of distal loop. However, only one (8.3%) of 12 cases with intestinal and mesenteric Tb, revealed positive findings on chest X-ray.
8. The engenital groups were associated with various congenital anomalies, and prognosis was more worsed.
9. In surgical management, short-circuiting duodenojejunostomy was performed in 19cases(79.2 %), in spite of marked difference in treatment between various leading causes of duodenal obstruction, and the majority of cases was impossible to removal of etiologic lesions.
10. Complications after surgery were wound infection in 6, cases (25%) and pneumonja in 2 cases (8.3%).
11. Postoperative results of duodenal obstruction were good in 18 cases (75%), fair in 3 cases,

and poor in 3 cases.
Generally, Malignant tumor in etiology was ,tworsed--in prognosis.
12. Operative mortality was one case of 24 cases (4.2%), of which cases was expired on postoperative loth day and annular pancreas in association with multiple congenital anomalies such as stomach perforation, hemivertebra, and atresia ani.
And these anomalies -maybe contributed to the cause of death.
